IMPORTANT GUIDELINES

• Prior appointment is essential.
• To make an appointment, please contact (091) 44 – 43584458 / 43584459  at least 48 hours in advance between 9 a.m. to 9 p.m. Alternatively you could send an E-mail at Health@raiyaonline.com
• Cancellations should be intimated at least 24 hours in advance.
• Please arrive at BM Raiya Clinics, before 9.00 am on the scheduled date of health Check-up.
• A minimum of 12 hours fasting and abstinence from consuming alcohol the night before is required. No food or beverage/milk etc. other than plan water should be consumed the morning.
• Please bring samples of stool and urine when you come for the check up. Containers can be collected from the clinic.
• Please bring all your earlier medical records when you come for the check up.
• Regular medication can be taken.
• If you are using spectacles, please bring them along
• A light break fast will be provided.
• Diabetic patients: No anti diabetic drugs should be taken in the morning. Please carry the drug/insulin, to be taken with breakfast.
• Please consult your regular Physician in case any special/additional investigations are required to be done and inform Clinic.
• Reports are available on the next day. Reports can be collected from BM Raiya Clinics Reception.
• In some schemes the reports are available later. We can courier the reports to the address provided by you. If so desired, with an additional payment

Instructions for collecting Stool Sample

• Collect morning's first stool sample in the container provided to you.
• Collect the sample with the help of spatula.
• After you collect the sample, close the lids tightly.
• Please place this container back in the plastic bag provided.

Handover this bag to the technician in the sample collection room. He will make labels for your samples for identification in process.
